Core Principles of Person-Centered Care for Elderly and Dementia Care
At Foxburrow Grange we place the principles of Person Centred Care first for all our residents. Person-centered care is a holistic approach that prioritises the individual's dignity, needs, preferences, and choices. By focusing on the person rather than the condition, this approach aims to enhance quality of life, maintain dignity, and promote well-being. Below are four core principles :
Individuality and Respect
Each individual is unique, possessing a rich tapestry of life experiences, preferences, and strengths. Recognising and understanding these differences is fundamental to person-centered care. Treating everyone with dignity, compassion, and respect, regardless of their abilities or limitations, fosters a sense of worth and belonging. It's about seeing the person, not just the diagnosis.
Partnership and Choice
Involving individuals and their families in decision-making empowers them and ensures that their preferences are respected. By developing personalised care plans and encouraging choices in daily life, we promote a sense of autonomy and control over daily life, such as meal choices, activities, and routines. This collaborative approach fosters trust and helps build strong relationships between care providers and individuals. It's about working together to create a care plan that aligns with the individual's goals and values.
Effective Communication
Effective communication is the cornerstone of person-centered care. It's about more than just exchanging information; it's about building relationships and understanding the individual's needs and concerns. Active listening, clear and respectful communication, and empathetic responses are crucial. By tuning into the individual's verbal and nonverbal cues, we can better understand their feelings, thoughts, and experiences.
Meaningful Engagement
Engaging individuals in meaningful activities that align with their interests and abilities can enhance their quality of life. Providing opportunities for social interaction, creative expression, and purposeful living helps maintain a sense of identity and purpose. By fostering a sense of belonging and connection, we can reduce feelings of isolation and loneliness. It's about creating opportunities for joy, satisfaction, and a sense of accomplishment.
Person Centred Care Activity Principles
Below is an overview of our elderly and dementia care activity principles and some of our tips.
1. Activity should have a sense of purpose and engagement
2. Always invite the person to participate
3. Offer choice whenever possible
4. Talk less; demonstrate more
5. Physical skills; focus on what the person can do
6. Match your speed with the person you are caring for
7. Use visual hints, cues or templates
8. Give the person something to hold
9. Go from simple tasks to more complex ones
10. Break a task down into steps; make it easier to follow
11. To end, ask: 'Did you enjoy doing this ?’
12. There is no right or wrong. Think engagement
